I understand that Pilates involves physical activity and risks, including strains, falls and aggravation of existing conditions. I consent to participation. The participant must follow the instructor’s guidance and stop exercising if experiencing pain, dizziness or feeling unwell. This declaration does not exclude any rights or liability that cannot lawfully be excluded.*

I explicitly consent to Engaged Pilates Clinic Sdn. Bhd. collecting, storing and using the participant’s health information to assess suitability, adapt sessions and support safe participation, with access by relevant authorised staff, as explained in the Privacy Policy.*
